Tax Exempt Form:
Use this form to submit to vendors to prove that purchases
are being made for a non-profit, and therefore are tax-exempt.
PRC Reimbursement
Form:
Use this form to submit to the PRC office any receipts
for which you require re-payment from the organization.

Post-Event Spreadsheet:
Use this spreadsheet to track the details of the event
in which you were involved. Submit completed spreadsheet
back to the PRC office.
Member
Participation Report:
Use this form to submit details about Member Participation
related to your committee or event. Please submit this form
within one week of the completion of member participation
activity to ensure proper recognition of members.
Non-Committee
Member Request Form:
Use this form only to request members from the Non-committee
Member List to staff an event. Please submit thi form no
later than 3 weeks prior to your event to ensure that your
staffing needs are met.
